Knot Tying Problem

Gents,
I am having a problem with the first knot (square knot with two half hitches) pulling down into a "V" when the second knot is tied.
First: the knots are tied on the outside and remain so. No hidden knots or cord. Why? Because that is the way it was done in the 1930's. On Bristol Aeroplanes.

The knot is as tight as I can pull, I have tried two loops, and then started the knot. No help.

Any ideas?
